I highly recommend the hosts listed below. I have either worked with them myself (hosting my own websites) or I have worked with them using a clients' account and found them to be superior.

KVC Web Host - After trying many others hosts, I have to say for the price this is one of the best hosts you will get and it's all unlimited hosting. Phenomenal and a highly recommended host.

Stable Host - A very highly recommended (privately owned) hosting company with respect in the industry. They offer low cost ($4.95/month to $8.95/month), as well as Enterprise level hosting (with 99.99% uptime guarantee via SLA), and VPS hosting too.

FWGuys - A very reliable and high-speed WordPress only hosting option that can give you speed & performance without any tradeoffs. You can request standard cPanel access if you require it.  Monthly payment options allow you to host 1 WordPress site for $10/month, or up to 6 WordPress sites for up to $20/month. Also includes a professional theme (Elegant Themes) in the price.

Media Temple - lightning fast hosting (*the fastest hosting we tested), at an affordable and reasonable price. They also offer cPanel, VPS and Dedicated hosting. 

CrocWeb - One of the fastest shared hosts; they run on SSD drives, and have similar features to other shared hosts without overloading their servers.  They do offer refunds if htey do not hit their uptime.

Cool Ice Host - An excellent European host recommended for eCommerce uses, especially Open Cart. If you are located in Europe this may be your best choice. They offer shared hosting, VPS and VPS hybrid hosting with a significant amount of horsepower.

Liquid Web - One of the better hosts in the USA, located in Michigan they have great support, reasonable prices and are known for their speedy connections.

SiteGround - A great choice for fast hosting. They offer shared, WordPress, Cloud and Dedicated servers to suit your needs.

A Small Orange - Hosting with a heart. This company cares about it's clients and does not overload it's servers. They are fast on service, and reasonably priced.  Their Knowledgebase is here including this great article on WordPress caching and how to optimize it with the LiteSpeed Nginx servers they use..

DigitalOcean - SSD Cloud servers starting at a reasonable $5.00/month, and they are based in New York city. I've not used them yet, but have heard only good things.

Site5 Web Hosting - You cannot beat their current 'free 30 day trial' offer. They have a CDN available, however their severs are not LiteSpeed servers, so a little slower. Check the status/uptimes of all their servers here.

Genius Host - Canadian data center host, with a great price. I cannot vouche for their reliability or service at this time. A low-cost alternative.

Fresh Roasted Hosting - I've heard only good things about this host based in Pennsylvania, USA with award winning customer support. Fast/uncrowded servers and reliable uptime (99.9%) make this a recommended host. They also offer VPS hosting, and Dedicated hosting.(September 28, 2015) No longer recommended, the speed of their MySQL databases slowed to a crawl after a recent move to new servers. They also do not allow relayed email sending (Amazon SES) unless you purchase a VPS server.

MDD Hosting - a fast LiteSpeed enabled host with really fast technical support as well. Worth checking out. Check the status/uptimes of all their servers here.(September 18, 2015) No longer recommended, speed accessing their websites slowed considerably after a recent move to new servers.

inmotion hosting - An excellent host with 24/7 technical service and astrounding uptime and reviews.(October 2, 2015) No longer recommended, they continued to state that my websites were taking up too much bandwidth, when they were not, and that I should purchase their VPS hosting (at a much higher cost). They could not prove I was using the bandwidth they stated, and then websites started getting tampered with so they would no longer run!

HostGator - They are my definite preferred host for most websites. Get excellent bang for your buck, and unlimited bandwidth, websites, as well as diskpace (choose the Baby plan.) Use the coupon: XSITEPROTEMPLATES  to get your first month for 1 penny! Services have returned to normal after a bit of a rocky stretch as they transitioned to new owners in February-March of 2014.(October 12, 2014) No longer recommended, they are also owned by EIG, service/support/downtimes have been terrible, there was one other final problem that ended our relationship, that was that I had some pages on my website that could not be viewed from other parts of the world! They would return a 403 error (Forbidden) for me and even for others I asked to view the page (for support purposes), the reply from their tech support (which I asked several times, continued to be: I understand you were having trouble with your site generating 403 errors. I logged into the server and parsed the log files for your domain and found no indications of 403 errors being recorded. I then parsed the access logs looking for you IP address and was not able to see any 403 errors being generated there either.  You can imagine, after more than 1 week of this, I had to move to another host, as they cannot even guarantee my website is viewable while on their servers.

BlueHost - You will find excellent bandwidth here for hosting videos if you need it. No longer recommended, they are also owned by EIG, service/support/downtimes have been terrible.

XSitePro VPS Hosting

If you need a little more 'juice', a VPS hosting environment can really give you the additional bandwitdth, horsepower, and freedom to setup any way you like.

WiredTree - Custom built, managed dedicated servers, and VPS web hosting that is reasonably priced and well supported. You cannot find a better place to host, a bit on the pricey side though. 'On average, our Support ticket response time is under 15 minutes for every customer all day.', and: 'Our uptime is 99.992% over the last 6.5 years.' They run a really great hosting blog, including posts such as this on Simple WordPress Tricks.

100 Terrabyte.com - Extremely reliable, fast and large dedicated servers available for a fair price. Excellent for running applications, including email servers for large volume sending.

Future Hosting - An excellent VPS host. I've used them and I have excellent support as well as uptime. You can choose your server location and pick one close to home, servers are available in NY, Chicago, London, Los Angeles, etc.

CybrHost - For shared, VPS or dedicated eCommerce merchant hosting I recommend this company based in Ohio. They are a bit higher priced, but worth it, if you demand quality service.

XSitePro WordPress Hosting

WordPress specific hosting will give your WordPress sites the acceleration that they need. Traffic Planet - Another fantastic host that supports WordPress only hosting, up to 5 sites for a reasonable fee. Check them out.

SiteGround - A great choice for fast hosting. They offer shared, WordPress, Cloud and Dedicated servers to suit your needs.

Lighting Base - Fast, reliable and fully managed WordPress hosting. I've not tried this one myself, but have had an excellent recommendation from someone that hosts multiple WordPress sites, that this is a great option.  Pricing is more reasonable then WP Engine (see above).

Pantheon - setup a free Developer account and two website, just to try it out. Very fast WordPress and Drupal hosting.

Pagely - WordPress for small business, an excellent way to go for high speed, reliable hosting. Plans are a bit more expensive here.

WP Engine - The best WordPress hosting period. Optimized page load times, reliability and security. Fast, Secure and Scalable.

easyPress - another WordPress based hosting with caching, CDN (content delivery network) and good prices built in.

Pressable.com - WordPress hosting plans, the 1-5 sites option is very reasonable.  CDN  is standard, and looks very powerful for the prices.

Use these tools to check host reliability, uptime, reviews and just about anything else.  It's important to fully research the host you will use to guarantee that they have the service and performance you need.

Namecheap.com - I now buy all of my domain names, and SSL certificates through them primarily because of their excellent customer support, and great prices. Well done.

Hyperspin - A great place to verify uptime on your accounts.  Use this link to see the most recent Hyperspin Verified Rankings of Current Hosts.  Worldwide monitoring is available, as well as simple host monitoring for single sites at a reasonable subscription.

Pingdom Tools - You can check the details of webhosts, and your own server using this powerful tool. This excellent collection of tools is perfect for checking up on your host. Use the full page speed test to get the nitty gritty details on any page loading times (that may affect your Google rankings).

GTXmetrix - The absolute best place to test your load times/speeds of your websites. Tests are performed from multiple locations and they include YSlow scoring as well.  The best when it comes to testing your webhost for speed.

Load Impact - A very thorough load testing website. Subscription required to run multiple tests.

LimeDomains - An inexpensive and good Domain Registrar (they let you register privately too, for reduced fees).

HostsMan - If you are doing anything at all with Windows and you want to test a host/server before you re-point your DNS to the new host, this utiility is a 'must have'. It gives you all the tools you need to safely edit your Windosts HOSTS file (which is hidden, protected, and locked otherwise). WebHostingStuff - Probably the best place to lookup a host and validate that they are good.

Review Hell - Brutally honest web host reviews, and coupons show up too, when you search. Great.

Web Hosting Talk - The #1 website for checking out what tech people have to say about hosts. If there's a host you want to checkout, make sure you search the forums here first.

SBI - SiteSell - An incredible resource which includes hosting, autoresponders, rss feeds, and a full Action Guide to help you get started in all areas of your online business. A nice way to get started, or something to look at if you want to return to the fundamentals.  Check out these public posts...

Web Host Magazine - Excellent articles, and reviews of all hosts. One of the best ways to find out about a host is to do a search through this website for anyting on the host your are reviewing, you can do that with Google Website search (Probably the best place to lookup a host and validate that they are good, enter webhostmagazine.com as the Site URL, and the host you are searching for as the Query).

Free Speed Test - Use the option on the right to enter a host company URL and then check their speed. *Pay attention to the Kbps number in the far right column.

Hyperspin - A better place to check real host uptime. Give it a try, see if your host appears, and then look at the quality uptimes listed from some of the other hosts here.

WebHostingReviews.com - One more place that you can lookup your host to verify that they are providing good service.

Find My Host - Another place you can look to find reviews and a report card on your host.  Also check the awards for the type of host you are looking up.
